In order to register at Elkmont High School, you must have the following documents:

•1.     Birth Certificate

•2.     Social Security Card

•3.     Original Blue Alabama Immunization Certificate(s) - This can be obtained from your physician or the local health department (no photocopies)

•4.     Proof of Residence

•5.     Proof of Legal Custody (if student does not live with both biological parents.)

•6.     Past Grades/transcripts

•7.     Name and Address of Prior School
